The company’s first product, CipherOutreach, focuses on hospital acute discharges and enables automated post-discharge follow-up call and text programs for transitional care management. It proactively addresses potentially adverse events through personalized patient care recordings and regular recovery checkups. Furthermore, it provides both point of care (POC) and remote patient engagement tools on the same recording platform, thereby uniquely covering the full patient continuum in the care journey from pre-arrival to POC to post-discharge and population health management.

CipherHealth collaborated with leading providers, health systems, and agile healthcare IT vendors for product co-development and periodic feature audits and enhancements after its initial solution launch. Its platform also gained secondary products such as CipheRounds, with a digital rounding application to support nurses rounding at patients’ bedsides.
